Care4Kids connects youth who experience out-of-home care with Wisconsin Medicaid coverage, plus tailored health care planning to help them get the most out of their benefits. With access to a health care coordinator, Care4Kids members get health, dental, and vision care that meets their wellness needs.
Now, Care4Kids is expanding these services to more children and young adults in Wisconsin:
- Children of Care4Kids members can get coverage, too. They don’t need to be in out-of-home care, and having coverage through Care4Kids doesn’t mean they are involved in the child welfare system.
- Youth up to age 26 who were in out-of-home care when they turned 18 can join Care4Kids. They must live in Kenosha, Milwaukee, Ozaukee, Racine, Washington, or Waukesha County.
- Care4Kids members in out-of-home care can live in more places and stay covered. Members in out-of-home care can move to any county in the Chorus Community Health Plan service area, as long as the county with child welfare jurisdiction is the Division of Milwaukee Child Protective Services, Kenosha, Ozaukee, Racine, Washington, or Waukesha County. Chorus currently serves all Wisconsin counties except: Burnett, Pierce, Polk and St. Croix.
- Youth in institutional or residential care can get care coordination up to 90 days before joining Care4Kids. Working with them early to get their health care organized can help make it easier to move to a placement in the community.
