Through a generous gift from Dr. Russell Libby, MD, FAAP, the American Academy of Pediatrics (AAP) desires to invest in a focused strategy to gain a robust understanding of value-based care and opportunities to inform policy, education and advocacy for value-based care. The Libby Fellow will work with the AAP staff and members to advance the AAP’s efforts to improve and innovate pediatric practice and to create value and quality care payment strategies that recognizes the unique value of pediatric practice to promote the health and wellbeing of all children.
Purpose
The purposes of the fellowship are:
- To identify ways to address challenges to value-based care and payment and innovate and advocate for public and private funding to support and sustain pediatric practice
- To leverage the Academy’s CHILD registry, quality initiatives, practice transformation activities, payment innovation and payer advocacy efforts to create a unified sustainable approach to practice transformation and modernization, advocacy for payment to support optimal care
- To identify opportunities for prospective interventions to reduce the impact of adverse childhood experiences and other ameliorable factors contributing to the evolution of chronic disease in adulthood to reduce incidence, morbidity, and cost of care
The fellow will create a body of work on pediatric practice and payment transformation to guide the AAP and its members to identify opportunities to support development, promotion, implementation and propagation of innovative and transformational primary pediatric care delivery that promotes value-based primary care and innovate and advocate for public and private funding to support and sustain pediatric practice. The fellowship report recommendations will be used to assist the AAP to define and create sustainable implementation strategies for policy, advocacy, education and training related to value-based care and payment and to augment and build on current AAP investments in equity and quality care.
To complete this work, the fellow will:
- Review relevant research on value-based care, primary care practice models, alternative payment models for both public and private payers
- Engage with the CHILD registry team to identify opportunities to align quality and registry work to drive indicators of value and quality in primary care that relate to Value-based payment
- Identify practice management and administrative structures (eg, staff, IT, care management, etc.) that add value and quality to care and determine viable and innovative payment mechanisms to support these structures under value-based payment models for primary care
- Engage, through key informant interviews and other methods, with AAP membership, pediatric and adult medicine experts in the field of health care finance and value-based care, state Medicaid officials, payers and others to present to the AAP a technical analysis and set of recommendations regarding value-based payment and other alternative payment models appropriate for pediatric practice and make recommendation for mechanisms to promote value-based care to members
- Work with the AAP Federal and State Advocacy team to develop recommendations for advocacy strategies for payment regarding value-based care and alternative payment models to include recommendations regarding mechanisms to promote value-based care to payers and members
- Collaborate with relevant AAP staff and members to develop a set of recommendations regarding sustainability of the Academy’s work related to value-based care
Fellow will produce CV worthy products including a report that contains:
- An assessment of the current landscape (Environmental Scan) of value- based care in pediatric primary care
- An analysis of the findings of the assessment to include, but not be limited to, payment model types, payment incentive strategies, utilization of team-based care, geography and practice type/setting
- Recommendations for potentially successful value-based care and other alternative payment models, policy options for pediatrics and actions for promoting/advocating various types of payment models
- Communication and advocacy opportunities at the practice, payer, federal and state levels to address challenges to implementation of value-based care and achievement of value-based payment
- Recommendations for implementation of various practice transformation models and practice investments in patient care, technology, education and payment and for sustaining the work processes related to implementation of recommendations
Fellowship Structure and Compensation
- The fellowship will be a remote, part-time (min .5 FTE) position for a 2-year period
- Annual stipend of $85,000 (additional funding will be provided to cover any needed travel costs)
- The fellow will report directly to the Sr. Vice President for Primary Care and Subspecialty Pediatrics
- The fellow must be able to invest the time necessary to meet the project deliverables while also maintaining their usual work obligations including the ability to meet with fellowship supervisor and other stakeholders. Occasional travel may be required.
Fellow Qualifications
- Ideal candidates should meet the following criteria:
- A medical degree, residency training and/or fellowship training, and board certification in pediatrics or a pediatric medical or surgical subspecialty
- A master’s degree in public policy, business administration, public health or health care financing is preferred
- Excellent oral and written communication and negotiation skills
